Why will Amazon conduct a third phone interview?

No, a third interview is pretty standard. It does not mean you did poorly in either round. If anything, the opposite is true. After all, why would Amazon do a third interview if you did really poorly before?

That said, I think you're looking at your performance the wrong way.
(1) It's not just about getting the algorithms "right." Did you find the optimal solution? After how long? How did you evaluate and discuss the tradeoffs?
(2) It's HIGHLY unlikely that you got all the algorithms correct immediately. That just doesn't really happen very often, unless you were asked all really easy questions. See Cracking the Coding Interview's discussion on how you are evaluated.

And just to be clear - you do not have to get every question right. You're evaluated relative to other candidates on the same question, NOT on an absolute % correct basis.
